We sincerely apologize to all residents living in the vicinity of our power stations and the general public for the tremendous inconvenience and anxiety due to the accident at our Fukushima Daiichi and Daini Nuclear Power Stations (the "Accident").
On this occasion, we would like to report that the fifth batch of application forms for compensation for damages shall be sent to all individuals. These application forms will be sent in accordance with our press release titled "Implementation of Compensation Associated with the Review of Evacuation Order Areas" and was announced on July 24, 2012.
1. Contents of Review of Claims from Individuals
(1) Introduction of a Comprehensive Claim Method (Please refer to "Appendix")
A compensation application form for damages is readily available for use (as announced on July 24, 2012) for all compensation items ("emotional distress" which includes cases of increased living expenses due to evacuation, "incapacitation damage," and "any other actual costs, etc.") as listed in the Appendix. Compensation for damage, which has accrued at any time during a specified period after June 1, 2012, shall be payable in one lump sum (hereinafter referred to as the "Comprehensive Claim Method").
For those individuals whose main place of residence was within the zone where residents cannot return for an extended period at the time the Accident occurred, compensation payable in the Comprehensive Claim Method shall be specified as such in the Appendix. For those individuals who resided within the zone under restricted entry or within the zone whose evacuation order is set to be lifted, compensation shall be paid out as accrued for the standard period until the prospective release timing of the evacuation instructions is decided. After a decision is made, additional compensation for damage shall be payable according to the relevant period. Furthermore, if the actual release timing of the evacuation instructions comes after the prospective release timing of the evacuation instructions, additional compensation for damage shall be payable according to the relevant period.
For those individuals whose main place of residence at the time the Accident occurred was within the areas for which a review of the evacuation order area *1 remains pending, compensation shall be paid as accrued during the specified period from June 2012 to May 2013. After a review of the areas is completed, additional compensation for damage shall be payable according to redefined areas or the prospective release timing of the evacuation instructions.
Compensation for incapacitation damage shall be paid as accrued for a period of 21 months *2 from June 2012 to February 2014.
*1 Evacuation Order Area: Any area that shall be treated as an "evacuation order area" as defined in the "Second Supplement to the Interim Guidelines for Determining the Scope of Nuclear Damages due to the Accident at TEPCO Fukushima Daiichi and Daini Nuclear Power Stations."
*2 The applicable compensation period for incapacitation damage is two years from March 2012. Claims for compensation accrued during the period from March to May 2012 the same year may be filed on the conventional type of (fourth) application forms.
Note 1. As for the use of the conventional types of application forms, those individuals opting not to use the Comprehensive Claim Method are kindly requested to contact the Fukushima Nuclear Compensation Consultation Room (Call Center) as indicated at the end of this document. In addition, once the Comprehensive Claim Method has been selected, application forms for the Simplified Claim Method shall no longer be available for use in the fifth and subsequent applications.
Note 2. Those individuals whose main place of residence was at any of the specific evacuation recommended points at the time the Accident occurred are kindly requested to use the conventional types of application forms for compensation for damages.
As for "incapacitation damage," "an amount equivalent to the actual expenses incurred for evacuation or coming home" and "an amount equivalent to the rental expenses incurred," if the actual cost (loss) incurred exceeds the total *3 compensation amounts paid for compensation items in the Comprehensive Claim Method, all necessary documents (receipts and certificates, etc.) shall be reviewed, and the excess shall be additionally paid (settled) to the extent necessary and reasonable.
*3 If payments are made collectively per household, the "total" means the aggregate amount paid to a household
Furthermore, the first acceptance of a claim for settlement of the compensation amounts is scheduled for around June 2013 (or around October 2012 restricted to those individuals who resided in the former emergency evacuation preparation zone). The specific settlement method shall be announced later.
(2) Calculation Method of Damages due to Impaired Work Capability (Application of "Special Effort")
As for the earnings obtained from a new employer through a job change or temporary employment, etc. at any time after the occurrence of the Accident, compensation for damage shall be payable without a deduction from the amount of "earnings that would have accrued had the Accident not occurred" for the applicable compensation period *5 after the 4th batch of application forms for compensation for damage (Applicable Period *4: From March 1 to May 31, 2012). The upper limit of the non-deductible amount shall be 500,000 yen per month. ("special effort": Announced on June 21, 2012)
*4 Applicable Period: Any period during which the claimed amount is finalized for each application for compensation for damage.
*5 Applicable Compensation Period: Any period during which the amount payable via one lump sum for each compensation item is calculated.
For the payment of compensation due to impaired work capability, the most recent employment status shall be reviewed. As for the status of employment as of the end of August 2012, those individuals who have been employed are kindly requested to submit supporting documentation verifying actual earnings obtained (e.g. a salary statement, or regular mail for pension funds), and those individuals who have been unemployed are kindly requested to submit supporting documentation verifying their status of unemployment (e.g. a Vested Recipient of Unemployment Insurance Benefits Certificate).
(3) Changing the Method to Claim Compensation for Life or Bodily Damage
Compensation for life or bodily damage shall be payable according to the special circumstances surrounding those individuals who have received medical treatment. Apart from the conventional types of application forms, different application forms are readily available for use from the release of the fifth batch of application forms for compensation for damage onward.
In addition, impaired work ability compensation claims due to life or bodily damage may be filed on application forms other than conventional types.
(4) Any Other Changes of Handling
For the fifth and subsequent compensation claims for damage, the method of handling shall be reviewed and changed with the settlement of actual costs in mind, as follows:

2. Dispatch and Acceptance of Application Forms
Application forms for the Comprehensive Claim Method shall be sent sequentially from September 27, 2012. The acceptance of these application forms shall start from October 3, 2012.
Application forms for life or bodily damage shall be sent sequentially from September 28, 2012. The acceptance of these application forms shall start from October 3, 2012.
Furthermore, for those individuals who agreed upon the 1st claim for compensation (Applicable Claim Period: From March 11 to August 31, 2011) and who filed the second and subsequent compensation claims, but have not yet agreed on the certain periods, direct mails shall be sent sequentially from October 4, 2012 meaning such individuals should be requested to file compensation claims for such yet-to-be-agreed periods.
In addition, those wishing to use the conventional types of application forms (Applicable Claim Period: From June 1 to August 31, 2012), or those who have changed their mailing address are kindly requested to contact the Fukushima Nuclear Compensation Consultation Room (Call Center) as indicated at the end of this page.
3. Others
Application forms for successive periods are readily available for use. These application forms are used to collectively file claims for periods not agreed to date with respect to the first through fourth compensation claims for damage (Applicable Claim Period: From March 11, 2011 to May 31, 2012). The schedules for dispatching and accepting these application forms shall be announced later.
