Third batches of compensation application forms to be sent out
  We sincerely apologize to the people living in the surrounding areas of 
the power station and broader society for the concern and anxiety that 
have arisen on account of the accident at Fukushima Daiichi and Daini 
Nuclear Power Stations (the Accident). 
 
We have been making every effort to compensate those who have been 
afflicted by the Accident in a swift and fair manner under the framework 
of Nuclear Damage Compensation Scheme that includes Nuclear Damage 
Facilitation Corporation Law. We announce today the third batches of 
compensation application forms to be sent out. The updates and additional 
information are as follows: 
 
1. Application from the Individuals 
(1) Introduction of Easy Claim Procedure (please see "Appendix 1" for 
common application flow) 
With regards to the application by those who were forced to evacuate from 
the designated evacuation zone*1 (Individuals), aiming to reduce paper works 
for swift compensation payouts, to implement the third application of 
compensation (compensation for the period from December 1, 2011 to February 
29, 2012), we start sending out new application by employing new system 
(Easy Claim Procedure), which, in principle, does not require submission 
of the claimed amount description and certificates. 
For the applicants wishing to require previous type of application forms, 
please contact "Fukushima Nuclear Compensation Consultation Room (Call 
Center)" described at the bottom of this announcement for details. 
 
 ■ Individuals eligible for the use of new application forms 
 Individuals who have agreed both the first and second compensations are 
 eligible to use new application forms. 
 
 ■ How to apply for compensation 
 A copy of application forms for each household will be necessary under 
 this new procedure, while we have had each individual to fill up the 
 application forms so far. 
 Compensation amount for each claimed item was set down in accordance with 
 the compensation amount actually paid. However, for the evacuees who 
 underwent changes in circumstance, please request a calculated 
 compensation on that basis. 
 
(2) Additional compensation items 
The following items are added since they are considered the damages 
consequential to the Accident. 
 
 (a) Actual expenditure to accommodate at houses of relatives and 
 acquaintances 
 ■ Eligible applicant 
 For the Individuals who accommodated at houses of relatives and 
 acquaintances during the period from March 11, 2011 to November 30, 2011 
 in connection with the evacuation from the designated evacuation zone. 
 
 ■ Eligible damage 
 Actual expenditure to accommodate at houses of relatives and acquaintances 
 during the period from Mar 11, 2011 to November 30, 2011 in connection 
 with the evacuation. 
 
 ■ Compensation amount 
 · 2,000 yen per household per night (standard case) 
 · Up to 60,000 yen per household per month 
 
 (b) Damages due to voluntary evacuation 
 Details of compensation for the damages due to voluntary evacuation for 
 those who held resident within the area of designated evacuation zone at 
 the time of the Accident are described below; 
(3) Delivery and receipt of application forms · Those who have agreed both the first and second compensations: We will send out Easy Claim Procedure application forms from March 9, 2012 and start receiving them from March 16, 2012. · Those who have NOT agreed the first and second compensations: We will send out information for commencing the third compensation from March 9, 2012. 2. Application from corporations and sole proprietors (1) Amendment for the period for compensation (please see "Appendix 2") In response to many opinions and comments, we decided to accept application for long-term compensations on top of the 3-month fixed damage compensation we had employed so far. Details of the period are specified below; · Application will be eligible on monthly basis for minimum of 3 months up to 12 months. However, in case of claims for the period from March, 2011 to March, 2012, we will accept 13-months applications. · For the application of the first compensation period (from March, 2011 to August, 2011) and the second compensation period (from September, 2011 to November 2011), a summed compensation amounts for the periods combined should be applied. · We will not accept application for the expected future damage. (for example; in the case of application for the damage of March, April and May, 2012 to be submitted in March, 2012 will not be acceptable.) (2) Addition of eligible damage for compensation (damage by harmful rumor for tourist industry) For the damages such as financial losses due to harmful rumors that afflicted corporations and sole proprietors in the tourist industry having their primary offices located in Yonezawa City of Yamagata Prefecture, we have included them in the compensation items as they are considered consequential to the Accident (previously announced on February 16, 2012) and we have start receiving application.
