On March 11 2011, turbines and reactors of Fukushima Daini Nuclear Power
Station Units 1 to 4 (Boiling Water Reactors, rated output 1,100 Megawatts)
that we had been constantly operating at rated thermal output were
automatically shut down at 2:48 pm due to the Tohoku-Chihou-Taiheiyou-Oki
Earthquake (previously announced on March 11).
At 7:15 am on March 15, the reactor of Unit 4 achieved cold shutdown. As a
result, all reactors of Units 1 to 4 at Fukushima Daini Nuclear Power
Station achieved cold shutdown (previously announced on March 15).
As of 3:00 pm on August 26, the reactors of Unit 1 to 4 are in cold
shutdown (please refer to the appendix). We will continue to make our
endeavors to further stabilize each plant of cold shutdown.

○ Results of Survey on the Injury of a Worker from a Partner Company in
the Service Building of Units 1 and 2
<Survey Results>
On June 23, 2011, one of the workers from a partner company, who was
supervising the conveyance of items at the checkpoint of the service
building of Unit 1 and 2, got his clothes caught by the buckle of a fire
extinguisher on the wall and got the fifth finger of his right foot
injured by the falling fire extinguisher and was taken to the hospital by
an ambulance. On June 24, he was diagnosed as "laceration of the fifth
finger on right foot, fracture of distal phalanx". (previously announced
on June 24).
<Survey Results>
The results of our survey are as follows.
·The supervision over the conveyance was conducted in a narrow space and
the fire extinguisher was placed on the wall nearby.
·The fire extinguisher was fixed with a band only and it could easily
fell off when the workers accidentally touched the band.
·The worker from a partner company was not aware that the fire
extinguisher might fell off.
·The worker wore a pair of socks while supervising the conveyance,
because the workers usually put on underwear and socks when they go
through the checkpoint of entrance into and exit from the controlled
area.
·There were no clear precautions (rules) regarding outfits for
supervising work of conveyance.
<Probable Causes>
There were no clear rules regarding outfits for supervising work of
conveyance at the checkpoint, as a result of which the worker was wearing
only a pair of socks. In addition, the fire extinguisher could easily fell
off when the workers accidentally touched on the band, therefore it fell
on the right foot of the worker who was not wearing the safety shoes while
he was supervising work of conveyance in a narrow space and his clothes
were caught by the buckle of a fire extinguisher. Those are the facts
which probably caused the injury.
<Countermeasures against reoccurrence >
·We expanded the area for supervising work of conveyance and secured a
safety distance from the fire extinguisher.
·We established a precaution (rule) about outfits for supervising work of
conveyance, which clearly obliges the workers to wear safety shoes.
·We placed a band with a hook to fix the fire extinguisher on the wall.
·We announced this accident in a various meeting which the partner
companies attend.
